Receiving medical care, working with your care team and dealing with insurance can be overwhelming. And sometimes, other parts of life – work, school, relationships, home life, transportation and access to healthy foods — make the next step in medical treatment even more complicated.
A free service for patients, our social workers help patients and families understand their health issues, make decisions, understand their rights and empower them to participate in their care.
Our licensed clinical social workers work across all our health system, from the emergency department to inpatient units and clinics. We also coordinate important services to help navigate medical care and life outside of the healthcare setting. Our efforts connect patients and their families to resources to help cope and take the next step after discharge.
We provide a number of support services, including:
- Transportation
- Helping cope with a diagnosis
- Medical equipment in the home
- Services to assist with rent or utilities
- Medicare and Medicaid planning and paperwork
- Arrangements for post-acute services
- Legal guardianship process liaison
- Coordination of counseling and mental health
- Substance misuse counseling
- Long term care
- Home health
- Help navigating our health system
- Help transition after discharge
